On one hand, you need to beat the time on Agent difficulty, which is the difficulty you usually play on when first discovering the game. On the other hand, while all you need to do is grab a key and get in the plane, the time to beat is 5 minutes. Since the level in a straight line takes less than 30 seconds, there is a good chance you obtained Big Head mode without really knowing what you had done.
